Word Life Yoga, LLC — General Liability Waiver & Release of Claims Instructor / Business: Wildlife Yoga, LLC (“Instructor”) Location: Various venues throughout Pennsylvania (“Venue”) IMPORTANT NOTICE: This is a legal document. By signing, you are releasing certain legal rights, including the right to sue. 1. Assumption of Risk I acknowledge that yoga, fitness, strength training, mobility work, and related physical activities (the “Activities”) involve risks, including but not limited to: Slips, trips, and falls Muscle strains, sprains, and joint injuries Cardiovascular events or overexertion Aggravation of existing medical conditions Risks associated with exercising at indoor or outdoor venues, including surfaces not specifically designed for fitness I understand and voluntarily accept all risks of injury or harm, whether known or unknown, even if caused in whole or in part by the negligence of the Instructor, Wildlife Yoga, LLC, or the Venue. 2. Health & Medical Clearance I affirm that: I am in good physical condition and able to participate safely in the Activities. I have consulted with a physician, or have voluntarily chosen not to, and I accept full responsibility for my health and participation. I will monitor my own condition during class or training and stop immediately if I experience pain, dizziness, or other concerning symptoms. 3. Release of Liability In consideration for being allowed to participate in the Activities: I, on behalf of myself, my heirs, executors, and assigns, hereby release, waive, and discharge Word Life Yoga, LLC, its owners, instructors, employees, contractors, agents, any venue owners or managers where the Activities are held, and all affiliates (the “Released Parties”) from any and all claims, demands, or causes of action arising from my participation in the Activities, including those caused by the negligence of the Released Parties. This release applies to claims of personal injury, illness, property damage, or wrongful death, to the fullest extent permitted by Pennsylvania law. 4. Indemnification I agree to indemnify and hold harmless the Released Parties from any liability, claims, demands, costs, or expenses (including attorney’s fees) arising out of my actions or participation in the Activities. 5. Severability If any portion of this agreement is held invalid, the remainder shall continue in full legal force and effect. 6. Understanding & Consent I have read this document, fully understand its contents, and voluntarily agree to its terms. I understand that by signing this waiver I am giving up substantial legal rights, including the right to sue the Released Parties.
